If you want to have a positive impact in the lives of others come join us! Elder & Adult Services provides in-home and agency clinical assessments and services to clients (Medicaid/Non-Medicaid) 21 years of age or older.
Elder & Adult Services utilizes the Gatekeeper Case Finding Model to proactively locate, identify, and serve clients in need of support and assistance in their home.
Elder Services serves as the primary access point for services and resources that promote independence and enhanced quality of life for vulnerable older adults who are at risk or isolated.
Services include: Case management Outreach to clients in their homes Individual therapy Group therapy Psychiatric medication management Peer support services The Mental Health Care Provider provides clinical behavioral health services to clients within their professional scope and collaborates with other care team members assigned to support client’s progress toward established goals and objectives.
Services include but are not limited to engagement, orientation for new clients, supportive listening, psychoeducation, behavioral health counseling, care management, skill building, crisis assessment, prevention and intervention, resource education, and referral to internal and external resources with individuals of all ages in virtual, office or community-based settings.
ESSENTIAL DUTIES AND RESPONSIBILITIES includes but are not limited to the following: 1.
Provide episodic, trauma-informed, person-centered counseling utilizing evidence-based practices within scope of practice.
2.
Maintain client records and all other required documentation in a timely and confidential manner in accordance with agency policies and procedures.
3.
Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and stabilization services as needed, including facilitating access to inpatient facilities for behavioral health care.
4.
Facilitate and co-facilitate client treatment groups.
5.
Represent the agency and its services to the community in a positive and professional manner.
6.
Participate in required staff meetings, including regular supervision and weekly clinical staff meetings.
7.
Participate in any required training, consultation calls, and onsite visits or reviews when required by contract.
8.
Accept delegation and follow through with specific expanded assignments and responsibilities within FBH system such as quality improvement teams, task groups, committees, consultations, mentoring or training responsibilities.
9.
Establish and maintain collaborative working relationships with clinical, administrative and support staff, community partners, clients, and family/natural supports.
10.
Identify and include the client’s support network and applicable involved community partners in care coordination.
11.
Serve as primary care team member and meet productivity standard in accordance with agency program guidelines.
12.
Develop and implement care plans with measurable objectives and obtainable goals, in collaboration with the client, evaluating clients progress and adjusting as needed.
13.
Refer clients to master level clinicians, peer support specialists, prescribers, care navigators, or other team members for additional treatment needs as identified on the care plan.
14.
Provide care management services to clients including: Assist clients in identifying and accessing resources and collaborate with community resources on behalf of clients.
Assist clients in the development of healthy coping mechanisms in individual or group settings.
Provide employment and housing assistance as appropriate and necessary.
Offer education on client diagnosis.
15.
Comply with all state WAC requirements and FBH policies and procedures.
16.
Perform FBH charting requirements, correspondence, Workday, and other paperwork in a complete and timely manner.
17.
Operate as a member of a multi-disciplinary team.
18.
May be assigned to supervise bachelors level interns.
19.
Other duties as assigned by agency or supervisor.
QUALIFIED CANDIDATES WILL HAVE: REQUIRED: Bachelor’s degree in social work, psychology, or a related field from an accredited college or university.
Must submit for Agency Affiliated Counselor Registration within 30 days of hire.
PREFERRED: One year of experience working with people with a psychiatric impairment.
Experience with crisis intervention.
KNOWLEDGE, SKILLS & ABILITIES: Knowledge of Washington Administrative Codes and statutes pertaining to the treatment and delivery of mental health services.
Must possess excellent communication skills, including the ability to send and receive oral and written messages, in a clear, positive, prompt, and appropriate manner, while maintaining customer confidentiality.
Demonstrated ability to apply active listening techniques conveying respect, professionalism, engagement skills, positive attitude, and empathy.
Some positions require travel between work sites or to see clients in the community.
Mental Health Care Providers working in such a position must have a reliable transportation, insurance, a valid driver’s license be willing to use personal vehicle in the course of work.
Demonstrated ability to apply active listening techniques conveying respect, professionalism, engagement skills, positive attitude, and empathy.
PHYSICAL, SENSORY & ENVIRONMENTAL: The demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

This position provided client services in-office and within the community at large on a occasional to frequent basis.
Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets or anywhere in the community at large.
Some environments may not be ideal in terms of cleanliness or being habitable.
Occasional frequent travel between locations is required.
The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required Daily use of a computer is required.
May be required to lift 10 - 20 pounds.
PAY RANGE (Hourly/ Non-Exempt): $25.
14-$28.
29* *All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay for this position is $25.
14 per hour.
This position is eligible for quarterly productivity incentives.
