Registered Nurse - Case Manager

*Registered Nurse- Case Manger* Maxmed Healthcare is seeking a Registered Nurse Case Manager to provide full-time services at Fairchild Air Force Base, WA.
Our Registered Nurse Case Manager: * Provides case management, care coordination and discharge/disposition planning for inpatient and outpatient care settings.
* The RN-CM assists service members and/or veterans, family members and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems.
* Coordinates care with multiple providers across all levels and sites of care.
* Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in multidisciplinary patient care management practice.
* Also evaluates care and outcomes to ensure timely and appropriate provision of services.
The CM will also be the alternate Special Needs Coordinator (SNC) at the Fairchild AFB.
* Degree: Associate Degree in Nursing (ADN) as a minimum required or Bachelors of Science in Nursing (BSN) * Experience: 24 months of experience within the last 48 months in Case Management * Certification: Basic Life Support (BLS) * Licensing: Contract personnel shall possess active, valid, current and unrestricted license to practice as a Registered Nurse in any of the 50 U.
